    Story

In 2025, in recognition of their past achievements, new MERs have been established in major cities across Japan: Sapporo, Sendai, Nagoya, Osaka, and Fukuoka. Meanwhile, Okinawa and Kagoshima are actively seeking MERs capable of serving remote islands.

Trial operations of the "Nankai MER" have begun, traveling to islands in the southern seas on a ferry carrying the NK1, a specialized vehicle equipped with an operating room. TOKYO MER Chief Doctor Kitami and Nurse Natsume Kuramae are assigned to Nankai MER as instructors, providing medical care to underserved remote islands.

One day, a massive eruption suddenly occurs on Suwanosejima Island in Kagoshima Prefecture. Lava engulfs the village, and huge volcanic rocks are hurled through the air, destroying roads and buildings.

Aerial rescue by helicopter is impossible due to the volcanic smoke and ash. And the time it would take for the Japan Maritime Self-Defense Force and the Japan Coast Guard to mobilize and get to the island is time the residents, many being children and the elderly, simply do not have.

In this desperate situation, the Nankai MER embarks on a desperate mission to save the lives of all 79 people stranded on the island.
